AP Compliance Form (Download PDF)
This form should be completed in its entirety when a requestor wishes to use a business’ goods or services but the university does not yet recognize the business as a vendor. It is imperative that this form is submitted as the university must operate within accordance with the Internal Revenue Service and the State of Ohio's regulations. The form must be faxed or mailed to the business and fully completed. Once returned, the completed form must be submitted to Business Operations Unit and Purchasing that the prospective business may be reviewed and approved. After the business is approved and recognized by the university, the goods and services it provides may then be rendered.

Blanket Purchase Order Form (Download MSWord document)
This form should be completed when a Blanket Purchase Order is needed. There must be justification for the Blanket Purchase Order. Valid reasoning includes emergency purchases or recurring purchases of a specific service or commodity when the quantity or frequency of delivery cannot be fully determined. The requestor must provide supplier contact information, a description of the services and the Blanket Purchase Order number. Further information that must be recorded is the date of the release, by whom the release is requested, an authorized signature, a description of the item including quantity and unit of measure, as well as an estimated cost. This form should be completed for every Blanket Purchase Order.

Purchasing and Travel Card Transaction Form (Download PDF)
This form must be used when one wishes to make a purchase with a purchasing or travel card. The requestor must obtain Pre-Approval from authorized persons with Valid Business Purpose before the form can be submitted. The requestor must also account for the Org, Fund, Account, Project, Program, User Defined and an Original Itemized Receipt with a Detailed Description of each Item. When traveling, a T-Number must be provided along with the aforementioned.

Journal Entry Approval Form (Download MSWord document)
This form should be used when one wishes to transfer funds from one account to another. The requestor must provide vital information on this document in order for transactions to be complete. A valid reason must be given as to why funds should be transferred.  This information should be communicated under the Description/Explanation portion of the form. In addition, the Org, Fund, Account, User Defined, and Program numbers from which the funds are located and to where the funds should go must also be provided and entered in the proper location. The requestor is responsible for signing the Initiated By line as well as for giving his or her contact information.  The form should then be signed and dated by the Supervisor of the requestor. This document must be completed in its entirety to ensure expedience.

ATF Approval Form (Download PDF)
This form should be completed ONLY by the PURCHASER of goods or services that have been rendered BEFORE a requisition has been built thereunto. The purchaser must provide his or her Name and Contact Information as well as the Vendor Name and the Date the goods or services were purchased. The purchaser must Give Account for why the Purchase Order was not requested before the purchase was made. The purchaser must also Sign the form and seek a signature from his or her Department Head. This form must show the requisition number and should be attached to the requisition when submitted.

Request for Payment Form (Download PDF)
This form should be used when one wishes to pay a vendor for any goods or services that have been provided. The consumer should supply all information for the vendor or PAYEE, including the Name, Last Four Digits of Social Security Number, or Federal Tax Identification Number, Address, Status with the University, and the Type of Payee which is located at the top of the form.

Furthermore, within the PAYMENT section, the Type of Payment with an Accurate Description should be accounted for. The requestor should also provide the Date, Location, Purchase Order number (if applicable), the Disposition of the Check, and the Amount requested.

In addition, the requestor is responsible for providing the Org, Fund, Account, User Defined, and Program numbers found in the INFO section of the form. Lastly, the requestor should give his or her Contact Information and Sign and Date the form.
