Assignment of University Equipment Form
Description: This form must be completed by staff members that are assigned a university owned desktop located at the staff members home, laptop and other portable computer equipment.

Data Security Access Request Form (MARX)
NOTE: You must have taken the Instituitional Data Policy (IDP) training BEFORE you will be granted access to MARX. To register yourself for the IDP training click here.
Description: This form should be completed by faculty, staff or graduate assistants who wish to access MARX screens on University Systems.
Instructions: Fill out the form by typing your information into the yellow highlighted areas. Once you have completed the form submit it to the MIS Systems Manager via email or campus mail. DO NOT Fax the form as it contains sensitive information.

Institutional Data Policy (IDT) Training Registration
Description: This online form should be completed by staff that need to take the IDP training but do not have access to the course on Carmen.

Network Logon Request Form
Description: This form should be completed by the supervisor of a new employee prior to the persons start date. Once this form is processed the new employee will receive their network logon. The process usually takes 7-10 business days.
